Lecture Lecture Outline:

I. INTRODUCTION

A. Describe the history and mission of your organization.

II. STATEMENT OF THE PROBLEM OR NEED

A. Describe the problem or need that your proposal seeks to address

B. State how your proposal will improve the lives of children. Include the ages and number of children.

C. Provide a brief narrative of the project’s geographic location, the people and their culture, and your relationship with the local and state government.

D. Include any data or statistics that would tend to support your application.

III. STATEMENT OF SOLUTION

A. List the goals or objectives of your project.

B. Include a description of your organization’s previous experience with other similar projects.

C. Provide a list of names and qualifications of key personnel who will be supervising or managing the project.

IV. FUNDING REQUEST

A. State the amount of your grant request and the purpose of the grant (in U.S. dollars).

B. Provide the projected cost of the total completed project.

C. Submit a detailed project budget, listing the amount of each item that needs to be purchased in U. S. dollars.

D. Include the anticipated project start and completion dates, and the approximate date when the grant funds will be needed

E. List other sources of funding for this project including funds previously received and funds pledged but not yet received.

V. FINANCIAL INFORMATION

A. Provide a copy of your organization’s Income and Expense Statement for the current year (year to date) and most recent fiscal year. A sample form is at end of this outline.

VI. EVALUATION AND REPORTING

A. Describe the methods your organization will use to measure the success of the project.

B. Provide a plan by which the project could become self-sustaining.

C. When this grant has been completed, what is the main source of income that will sustain this project?

VII. REQUIRED SUPPORTING DOCUMENTS

One (1) copy each of the following documents must be submitted with the grant application:

1. Letter from the Internal Revenue Service approving your organization’s 501(c)(3) status, or

· a copy of your organization’s listing in the current edition of the Cumulative List of Charities Published by the U.S. Department of the Treasury, or

· a copy of the page listing your organization in the current edition of the Catholic Directory, or

· a copy of the page listing your organization in the current edition of the Free Methodist Directory or other Protestant Denomination Directory that has a group ruling of tax exemption from the Internal Revenue Service of the U.S. Department of the Treasury.

2. If your government registers charities, you must provide a copy of the letter or ruling that confirms charitable status.

3. Letters of Recommendation:

All letters of recommendation must be from individuals who are familiar with your organization and the project. Letters from organizations should be on their letterhead.

· One (1) letter of recommendation if this is a project located within the U.S.

· Two (2) letters of recommendation if the project is located outside the U.S. including one from the highest religious or government authority in your province or community.

4. Your organization’s most recent Audited Financial Report, if available.

5. Your organization’s most recent Annual Report, if available.

6. The names and affiliations of your organization’s Board of Directors.

7. List of key personnel with qualifications.

8. Your organization’s website.

VIII. ADDITIONAL INFORMATION

A. Newspaper articles, newsletters, photographs and videos (optional)

B. Construction or renovation projects. Please include a detailed quotation on letterhead from the company doing the work. Describe how you will equip/furnish the completed project.

C. Equipment/Supplies. Please include vendor quotations on letterhead in U. S. currency. Describe where the item or items will be purchased, how it will be delivered to the project site, and the security of the item(s) once in place.
